$100 for completed HRA

CarePartners of Connecticut will continue to pay $100 per completed HRA for all enrollments with a 2024 effective date. Details are provided below:
Ways to Submit
- Connecture (if you have access) or
- Paper HRA (fillable pdf) – download from broker portal or email Medicare broker support.
- Please submit completed HRA paper forms to tmphra@point32health.org (only for paper HRA).

Please Note
- HRA must be completed at the time of enrollment or within 7 days of submitting the application
- HRA fee will be paid out with the commission of the application
- Ex. If an HRA is completed in October for a 1/1 member the fee will be paid with January commissions
- HRA fees will not be paid for any cancelled applications
- HRA fees will be recouped for any rapid disenrollments
- Applicable for Plan Changes
