Shop in 2024
Get up to $150 toward the full retail price of frames, prescription lenses, a combination of frames and prescription lenses, and/or contacts from a provider in the EyeMed network, or $90 toward glasses and/or contacts from an out-of-network store.
- $150 EyeMed network discount - More than 26,000 eye care providers make up the EyeMed network, including national chains like LensCrafters, Sears Optical, Target Optical, and JCPenney Optical. Present your member ID card when making a purchase, and $150 will be deducted from your total cost.1
- $90 out-of-network discount - After you make a purchase from an out-of-network eye care provider like BJ’s, Costco, or Walmart, you can receive a reimbursement up to $90 by submitting the Out-of-Network Vision Services Claim Form from the “Forms” page of our website.2, 3 You can also get the form by calling Member Services.

Preferred HMO members get up to $300 and Access PPO members get up to $250 toward the full retail price of frames, prescription lenses, a combination of frames and prescription lenses, and/or contacts, including upgrades.
- More than 26,000 eye care providers make up the EyeMed network, including national chains like LensCrafters, Sears Optical, Target Optical, and JCPenney Optical. Present your member ID card when making a purchase, and your discount will be deducted from your total cost.4
- If you make a purchase from an out-of-network eye care provider like BJ's, Costco, or Walmart, you can receive a reimbursement up to your benefit amount by submitting the Out-of-Network Vision Services Claim Form from the "Forms" page of our website.2 You can also get the form by calling Member Services. In- and out-of-network discounts cannot be combined.
